For Donor Advised Funds:

CNCF Giving Fund Application - to open a CNCF donor advised Giving Fund

CNCF Legacy Fund Application - to establish a CNCF donor advised Legacy Fund

Contribution Form - for additional contributions to any CNCF donor advised fund

Distribution Recommendation Form - for fund advisors only

Direct Transfer Form - to donate appreciated securities to a donor advised fund

For Pass-Through Gifts:

Direct Transfer Form - to transfer securities or mutual funds for liquidation and distribution by CNCF

Letter of Direction for Direct Transfers - outlines donor recommendations for the distribution to charities of proceeds of liquidation of transfered securities and mutual funds

For Bequests / Wills:

Letter of Direction for Testamentary Gifts - outlines donor direction for distribution of gifts received through a bequest or direction in a Will

For RRSP or RRIF account donations:
